The first is the only randomised controlled trial of observation alone versus surgery for prostate cancer, which was conducted in Scandanavia and which showed that surgery reduced the risk of death from prostate cancer at 10.8 years by 6% (Bill-Axelson, 2008). The second source are the 3,739 American men with prostate cancer who were managed by observation, radiotherapy or surgery, who were followed up for an average of 13.3 years and whose survival rates were published by Albertsen in 2007, showing the superiority of surgery over radiotherapy; both were superior to observation.

Active surveillance

Men with low-risk prostate cancer are likely to be offered surveillance, rather than definitive treatment, on the basis that not all men with prostate cancer will require treatment before their natural life span is over. Inevitably, there are both advantages and disadvantages to this option. Advantages: no side-effects. Disadvantages: the tools used to monitor prostate cancer progression (PSA and repeat biopsy) may not detect cancer progression in time to allow cure; anxiety; the need for repeat prostate biopsy.

High intensity focused ultrasound (HIFU) and cryotherapy

These modalities aim to damage the prostate by heating (HIFU) or freezing (cryotherapy) it, whilst minimising collateral damage and so reducing the risk of incontinence and impotence. Disadvantages: 80% of prostate cancers are multifocal (occur throughout the prostate); the absence of a positive prostate biopsy from that sector of the prostate does not mean that cancer is not present in it; larger cancers within the prostate are not always either the most aggressive ones or the ones most likely to spread; treating half the prostate may make follow-up of the other half more difficult by lowering the PSA level.


Radiotherapy can be delivered from outside the patient (external beam) or from within the patient by inserting radioactive seeds into the prostate under ultrasound guidance (brachytherapy). Both work in the same way but brachytherapy aims to deliver a higher dose to the prostate and less collateral radiation to surrounding structures (principally the bladder, rectum and erectile & continence mechanisms). Disadvantages: no final grade or stage assigned to the prostate; lymph node staus remains unknown; no target PSA; recognition of recurrence of cancer is delayed; surgery is rarely possible if radiotherapy fails; suggestion that pelvic radiotherapy doubles the risk of developing bladder and/or rectal cancer; more bladder irritation than surgery; more rectal irritation than surgery; tendency to pass blood from the rectum and in the urine due to radiation damage to the rectum and bladder; 11% % of men need to self-catheterise to dilate stricture (narrowing due to scar formation) in the urethra (Sullivan, 2008).

Advantages: final grade, stage and lymph node status allow precise calculation of prognosis; extended lymph node dissection appears to improve prostate cancer cure rates; unrecordable post-operative PSA blood test = cure; relieves urinary symptoms due to prostatic obstruction; recurrence is promptly detected by a detectable PSA; radiotherapy following surgery is much better tolerated than the other way round; equivalent or better cancer control, continence and potency in select patients in expert hands; allows de-bulking of high-risk prostate cancers (bulky and/or aggressive) as part of multi-modal treatment. Disadvantages: higher risk of stress incontinence than radiotherapy; more operator dependent than radiotherapy.
